Since 2021, the Jersey City Medical Center Food FARMacy program has been screening patients for food insecurity and chronic conditions that can be managed through nutrition (like diabetes, hypertension, GI issues, etc.). Qualifying patients are eligble for FREE nutrition counseling and FREE groceries, such as fresh fruits, vegetables, and meats, for themselves and their entire household. To date, the Food FARMacy has screened more than 1,850 primary care patients for food insecurity and has delivered about +8,000 bags of food to community members in Hudson County.
