Honoring your loved one through memorial giving
Monument Health Home+ Hospice is honored to provide compassionate care to our patients and their families. Many families choose to give back as a way of honoring their loved ones and supporting the continued care of others. We invite you to consider one of the following memorial giving options, each offering a meaningful way to remember your loved one while helping us continue our mission.
Memorial Giving: A Lasting Tribute
A memorial gift is a meaningful way to celebrate the life of your loved one while supporting the important work we do for future patients and their families. Your donation helps us continue to provide exceptional hospice care, ensuring that each patient experiences comfort, dignity, and peace. It also allows us to offer additional services such as grief support, family counseling, and community education.
General Donation
One of the best ways to give is through a general gift to Hospice. These donations help us respond immediately to priorities of Hospice and also allow us to utilize the money in the areas of greatest need. Gifts of any amount can be made as a one-time contribution or as part of a recurring giving plan. Complete the form on the left to make a general donation to the Hospice fund of your choosing.
